If not paid by Direct Deposit, we'll need copy of pay stub or other verification of income.

* Personal Check marked void (to ensure correct ACH funds transfer)

* ACH Authorization Form (viewable after application page submitted)

Upon completion of application and clicking I accept, the agreement signature / ACH authorization form will be displayed on your screen -please print and sign. Fax these required confirmation items to our toll-free fax number 1-877-369-6702.

A customer service representative will contact you of your approval (normally within one hour). Correct work and home phone numbers are necessary for approval and funds transfer.
